About the Hospital in the Home

Hospital in the Home shifts traditional acute care to the patient's own home, without compromising their care. This may take place in person, or increasingly, via the HITH virtual ward, utilising video Telehealth and wearable technology. Care in HITH is becoming increasingly acute thanks to the increase in access to technology. HITH programs have been shown to improve patient satisfaction, reduce costs per episdoe of care, reduce complications and improve overall hospital flow.

About the role

Austin's Hospital in the Home program prides itself on being an innovative service, striving to ensure people are receiving the right care, in the right place.

There are 3 key deliverable pieces within this projec role; 

  • Development of appropriate referral pathways and policy development to support suitable paediatric patients moving into The Northern Paediatric HITH Service.
  • Developemt of policies and pathways required for access of VVED (Victorian Virtual Emergency Department) consumers to be referred directly into Austin Health's HITH or virtual care ward for those within the Austin Health catchment.
  • Expand access pathways for Austin Health's Emergency Department including Short Stay Ward (SSW), to directly refer patients into virtual care/HITH.
