Position Description

A vacancy exists for a suitably qualified Case Manager to join our dynamic and Home Care Packages team to provide care coordination of the Transition Care Program (TCP). You will be coordinating high quality TCP services in association with other clinicians, the client and family/carers in the community and acute hospital settings.
The Transition Care Program (TCP) Case Manager provides service coordination to frail older people, and younger people with early entry approval to the Aged Care system, and their carers, to support them to remain at home and in their communities. The TCP Case Manager requires skills and/or experience in aged and community care, assessment and goal directed care planning based on a patient centered model of health.

The successful applicant will demonstrate the following attributes:
• Relevant tertiary qualifications and experience
• Demonstrated empathetic, respectful and professional attitude, adhering to high ethical standards.
• High level IT, decision-making and communication skills, including the ability to work autonomously and meet deadlines.
• Demonstrated case management, care coordination and goal directed care planning skills.
• Demonstrated understanding of rehabilitation and the principles and guidelines of the TCP program.
• Effective networking and mutli-disciplinary team liaison capabilities
• Experience working with aged consumers
• Thorough understanding of the range of issues experienced by and services relevant to this client cohort.
