Part time hours

Work 3-4 days (negotiable)

45-60 hours per fortnight 

The position will be based in the Gippsland region

There is some travel associated with the position

$41.52- $44.68p/hr (depending on skills/experience)

About the role ... 

We have a unique permanent part time opportunity for a Care Finder to support vulnerable older people who would not be able to arrange services without intensive support and do not have a family member or friends who can help.  

The Care Finder will be required to work both independently and collaboratively with stakeholders and partners to assist clients to navigate the service system.  The Care Finder will try to understand the person’s situation and support them to through the steps to address their needs.  Some of the key responsibilities of the role include:

  • Provide high quality needs based case-management support to eligible clients in consultation with client and where necessary other relevant family members and professionals, develop a case plan identifying key actions, responsibilities and timeframes needed to achieve agreed outcomes.
  • Liaise and refer to external service providers and other relevant support services within and external to organisation.
  • Monitor and report on KPI’s and activity targets and ensure they are met, taking corrective action when necessary in consultation with Manager
  • Ensure accurate client records and data is recorded and maintained in a timely manner as required
  • Ensure all reporting requirements are fulfilled as per funding body’s requirements
  • Liaise and report to Manager around program and provide reports as required
  • Keep abreast of changes in the Program and provide updates to Manager and relevant staff

About you ... 

You have have empathy, understanding and the willingness to assist vulnerable people.

Qualifications and Experience

  • Appropriate tertiary qualification, preferably in Social Work/ related Social Science

Skills & Experience

  • Demonstrated experience working with clients with complex needs
  • Demonstrated case management and assessment experience
  • Knowledge and understanding of the ageing process and the needs of people of CALD background.
  • Excellent communication, report writing, administration and time management skills.
  • Demonstrated ability to work independently and in a team environment.
  • Ability to think strategically, establish work priorities and manage competing demands
  • The ability to speak an additional language to English, and have a good understanding and experience of other cultures preferred.
  • Knowledge of mental health and or addiction conditions an advantage

A police check with no disclosable outcome, evidence of current flu vaccination and full COVID-19 vaccination is required by MiCare for this position.
